About Documentation
Documentation helps Disability Resources (DR) understand how your disability may impact you in an academic setting and determine appropriate and reasonable accommodations.
You can begin the process without documentation. If additional information is needed, our team will work with you to identify next steps.
What Counts as Documentation?
Documentation can come from a qualified professional familiar with your condition, such as a:
- Medical provider
- Psychologist or psychiatrist
- Licensed counselor or therapist
- Educational professional (for example, evaluation reports from a school)
Documentation should be current enough to reflect your present level of functioning.
What Should Documentation Include?
When possible, documentation should include:
- A description of the condition or diagnosis
- How the condition affects you in an academic environment
- Relevant functional limitations (for example: difficulty concentrating, reading, mobility, or processing information)
- Recommended accommodations (if available)
Not all documentation will include all of the above. DR will revie what you provide and follow up if additional information is needed.
If You Don’t Have Documentation
You can still request accommodations.
After you submit a request:
- DR will meet with you to learn more about your needs
- We may discuss options for obtaining documentation if needed
- In some cases, temporary accommodations may be considered while documentation is being gathered
Our goal is to reduce barriers—not create them.
Previously Used Accommodations
If you had an IEP or 504 Plan in high school, or accommodations at another college, you are encouraged to share that information.
This can help guide the conversation, but additional documentation may still be needed.
Confidentiality
All documentation is kept confidential and is only used to determine appropriate accommodations. Information is shared only as necessary to support your approved accommodations.
