NEW!!! The Multimedia Request Form is now available online! Click the link at the bottom of this page, complete the form online, and click the "Submit" button. It's that easy!
Multimedia Equipment is reserved on a first-come, first-serve basis, so please try to reserve/request equipment at least 3 days in advance. A hard copy of the Multimedia Request Form can be found in the Student Computer Lab/LSC (4th Street) and in the TLC (Room 458, Lone Tree). All equipment must be reserved in advance. Please do not move equipment from room-to-room, unless arrangements have been made with the TLC staff (x4368).
A Multimedia Request Form must be completed prior to checking out any equipment.
At the 4th Street Campus, all equipment must be picked up from the Student Computer Lab and must be signed out on the “Check-Out” form. When finished with the equipment, it must be returned to the Student Computer Lab and signed back in.
At the Lone Tree Campus, all portable equipment (digital cameras, laptops, etc.) must be signed out in the “Check-Out” binder which is located in the Teaching & Learning Center (Room 458). When the equipment is returned, it must be signed back in. For larger equipment (Tegrity, TV/VCR/DVD carts, etc.), you can pick it up from the Teaching & Learning Center or request delivery.
* At this time, students are not allowed to check out equipment for use off-campus. Students must arrange for use of equipment through their instructor or by contacting Hope Richardson at (928) 226-4229.
For questions regarding the use of Multimedia Equipment, or procedures, please contact Hope Richardson by phone at (928) 226-4229 or by email.
