MEDICAL, DENTAL, VISION AND LIFE INSURANCE

 

420.6.1 Purpose

 

This purpose of this benefit is to allow employees and their families to manage their health in a pro-active and cost effective manner.  The College’s medical, dental, vision and life insurances are coordinated through the Northern Arizona Public Employee’s Benefits Trust.  The scope and terms of the College’s insurance plans may be changed from time to time, and plans rescinded, notwithstanding the terms of this procedure.

                                                                 

420.6.2 Eligibility

 

The following employees are eligible to participate in the College’s medical, dental, vision and life insurance programs:

                                                  

1.       Regular full-time employees working 30 or more hours per week in a position that is expected to last no less than six consecutive months.

2.       Full-time one-year temporary employees (as defined in 443.1.3.3, #1).  Also one-semester, fulltime, temporary faculty are eligible for these benefits.  For faculty, one-semester is defined as equivalent to a six-month (half-year) period for staff employees for the purposes of benefits.

 

420.6.3 Scope of Benefit

 

The scope of each benefit is defined in each individual plan document.  Plan documents are available from the Human Resources Department.

 

420.6.4 Effective Dates of Coverage

 

Normally, benefits begin at the start of the month following the first full month of work.  For example, if an employee is hired on February 20th, his/her benefits will be effective on April 1st.

 

For staff, professional, administrative and temporary/full-time employees, benefits end on the last day of the month during which an employee resigned.  For full-time faculty who resign or retire at the end of a normal contract period, benefits extend through the last day of August.

 

420.6.5 Mandatory Participation

 

An eligible employee may not opt out of the College’s insurance programs.  Participation is mandatory.

 

420.6.6 Enrollment Periods

 

An annual enrollment period for health, vision and dental insurance is held during December each year. 

 

During the enrollment period, an eligible employee may add or drop dependent coverage from any of the insurances. Other than the enrollment period, an employee may make changes if the employee experiences a qualifying event, such as the birth of a child, change in marital status, etc.

 

 

 

 

420.6.7 Cost of Insurance

 

The extent of cost sharing between the College and the employee for single or dependent coverage premiums associated with medical, dental, vision or life insurance are determined by the District Governing Board.
