Setting up Email Forms on your Web Pages
Front Page allows you to set up web forms so that they
will send information submitted to a specified email address. This template focuses on
creating a simple Feedback Form and setting it up to email the results to a specified
email address.
Feedback Forms can be used for many purposes, including:
1) On-line tests and quizzes
2) Registration Forms
3) Guest Books
4) Check-out Forms
5) Order Forms
and many more!

Steps to Setting Up a Web to Email Form:
- 1) Open your Front Page Editor.
2) Click on New in the Files Option located in the Task Bar.
3) Click on the Feedback Form template.

4) In the form area, Right Click on one of the blanks in the form. Scroll down to Form
Properties and Double Left Click on it.

5) In the Form Properties window, type in your email address in the E-mail
Address: blank. Then Click on OK.

You can modify other variables, including: E-mail
Results, Confirmation Page, and Saved Fields
via the Options button. You can also customize your Feedback Form
web page by adding in images, shared borders, etc.
6) Spell check your document.
7) Save the file to your local computer (hard drive).
8) Publish the new file via the "Publish"
or "Import" option to the CCC Web Server.
Important Notes:
1) Do NOT set-up your feedback form to Send to: File and Email Address.
2) Only specify one email address per feedback form. If you type in more than one
email address, the message will NOT be sent.
3) This type of form CANNOT be tested in your Personal Web Server.
You must transfer the file from your personal computer to the CCC Web Server in order to
test the form. It is recommended that you DO NOT link to the form
until you have thoroughly tested the form to make sure it works.
